The Artistry in Music Scholarship Foundation
Financial Aid
Program
The Foundation raises tuition funds for students whose
financial resources are limited.
This
will enable such students to study music when the tuition would otherwise be
more
than they could
afford. The financial aid budget is small, but every effort will be made to
offer assistance to
deserving applicants.
Financial
aid from the Artistry in Music Foundation is based on the applicant's monetary
need. A scholarship
committee reviews applications for financial aid. The committee
will meet before
the beginning of each session of instruction to review applications and
designate award
amounts. Applicants are notified immediately afterwards. Please
contact the Foundation
at (847) 524-3696 for information on application deadline dates.
Initially,
financial aid is to be awarded for a weekly lesson, one-half hour in length,
for a
period of three
months. If a student cancels instruction, financial aid is forfeited; the
student must re-apply
upon returning. Awards will be granted on a sliding scale
determined by household
income and special circumstances (see below). Aid is
renewable for
subsequent sessions upon review by the committee.
A
financial aid grant is applied only to the enrollment requested by an applicant
on the
application form.
Subsequent changes-enrollment of additional household members or
addition of a class,
increase in lesson time—are not covered. The committee will
consider
re-applications to include such changes at its next meeting (to become
effective the following semester). Recipients of financial aid are expected to
demonstrate commitment to their musical studies by consistent effort and
regular attendance. The student must
notify the instructor prior to the scheduled lesson in order to be excused. Without such notification of extenuating circumstances and a valid
reason (such as family emergency, sudden illness, etc.), an absence will be
considered unexcused.
Scholarships will be rescinded following four or more consecutive
unexcused absences, or three or more absences per instructional quarter.
Criteria
used in determining aid awards:
1. Information
provided on the financial aid application.
2. Special
circumstances such as job loss, divorce, sudden medical expenses, bankruptcy,
etc.
3. Number of students
in the household taking private music lessons/dollar amount of tuition per
session.
4. Number of people
in the applicant's household.
5. Account
payment history (if applicable).
Because of the many factors involved in the committee's
decision making process,
recipient’s household
income levels may cover a wide range. While households of any
income are welcome
to apply, we do request that those in higher brackets only do so if
they are genuinely
in need of assistance.
Filling out your application:
It
is critical that all portions of the application form are completed. The
committee will
not consider
incomplete applications. If a portion of the form does not apply to your
circumstances, simply state
"N/A". This way the committee is assured you did not
overlook that
information.
PLEASE PRINT LEGIBLY THROUGHOUT THE APPLICATION.
Father/Mother
information:
Adults can disregard this line, but please state "N/A" and
fill in your
address, telephone, and occupation.
Special circumstances: Provide as much information and
detail as possible. Often your financial documentation does not truly reflect
your current fiscal situation (as in the case of sudden illness, pending
divorce, etc.) Use additional pages if necessary. The more information provided
to the committee, the better informed its decisions will be.
Signature: The application is incomplete and will
not be considered without your
signature.
Tax returns:
We must have formal documentation of your income. Please attach
your latest IRS 1040 or 1040A form (and your Schedule C if you
are self-employed
or run your business) to the application. If you
receive welfare, Social Security, or
other recurring
government payments, provide a copy of your notice of benefits
(including the actual dollar amount) or a payment check stub.
The committee must have
as much
documentation as possible and will not consider incomplete information.
Financial
information: Please complete each portion, writing "N/A" if
not applicable to
your situation.
ALL
INFORMATION PROVIDED ON OR WITH YOUR APPLICATION IS
HELD
IN STRICT CONFIDENCE.
